If you are preparing for your first appointment, please complete our Personal Checklist and make sure to bring it as well as your Judgment of Dissolution and any modifications to the Judgment. It is especially important to bring the current order for support and/or custody and visitation.

During the settlement process, you may be asked to complete a form(s) for our office or for the court. You will find a .pdf version of those forms below. Please download and complete the necessary documents, returning them to our office by fax or email.
